Directions: The
information requested on this form is essential for academic program
review. In some cases, you will provide
the information directly on this form; in other cases, you will provide the
information as an attachment.
1.
Include the department’s Strategic
Plan and label it Attachment 1—Strategic Plan. Note. Before
completing Section 2, update your mission statement (if necessary) in
UAOPS. Also update the unit profile for
your department as a whole, and update the profile for each of your degree
programs. (For UAOPS assistance, the
contact is Julie McAdams in OIRA). 2. After
revising UAOPS, print the current department mission statement, department
profile, and each degree program’s profile.
Include the mission statement and profiles in this report as the second
attachment, labeled Attachment 2—UAOPS 3.
Attach a copy of your department’s tenure and promotion criteria and label it Attachment 3—Tenure and Promotion Criteria. 4. Faculty Assignment by rank for most recent
fall semester
5. List
the number of faculty publications
in refereed scholarly journals over the last 5 years, and compute the ratio of
publications to faculty. (This section may
be modified appropriately if scholarly journals are not of great importance in
the field and are not a high priority in the department’s tenure and promotion
guidelines.)
Additional Publications (This section also may be modified, as noted
above, to reflect
publication priorities in the discipline)
6.
Indicate the ratio of the total number of citations in journals to the number of faculty over the last 5
years. (Citations appear in such sources
as ISI's Science Citation Index, the Social Science Citation Index, and the Arts
& Humanities Citation Index). If citation indices are unavailable in the
discipline or computation of citation ratios is misleading, provide any other
evidence you may have of the frequency of faculty research citations. If appropriate, also include awards from the
NEH, NEA, Guggenheim Foundation. American Foundation, American Council of
Learned Societies, and other sources. 7.
Briefly describe the undergraduate and graduate teaching loads and advising
responsibilities of faculty for the past academic year. |
