IMPORTANT: Before you submit your application materials, please carefully review the appropriate application instructions and checklist.
Application Forms & Supporting Documents
  • Graduate Application Form: Submit your graduate application form online:

  • Application Fee: You will have an opportunity to pay your application fee while filling up the online graduate application form, linked above. However, if you intend to mail your application fee to the Graduate School, you must submit the Payment form along with the application fee (Note: Application fee is waived for expedited applicants and McNair scholars)

Transcripts: Request two (2) official transcripts from each institution of higher education attended be sent directly to the Graduate School at: The University of Alabama Graduate School, P.O. Box 870118, Tuscaloosa, AL 35487-0118 (For express or courier mail services, use the physical address: The University of Alabama Graduate School, 801 University Blvd, 102 Rose Administration, Tuscaloosa, AL 35487-0118). For international applicants, please visit country specific credential requirements.

  • Manage other supporting application documents online: Recommendation Letters, Statement of Purpose, Resume, Professional Statement, Writing Samples and other documents (except transcripts) as required by your department:

    NOTE: Although it is highly recommended that you submit and manage your supporting documents online, yet under special circumstances you may directly send your documents to Debbie (for U.S./Permanent Residents) or Rebekah (for international applicants) by email attachments. Also, if your recommender is unable to submit the letter online, you may direct him/her for mailing paper-based recommendation letter by following these instructions.

Health Center Forms

  • The University Health Service requires all entering students to have a medical clearance. At the time of registration, you must present proof of a measles, mumps, and rubella (MMR) vaccination. Click here for a list of medical forms.
