Use this form for all assistants who: (1) will have primary responsibility for teaching a course and/or assigning final grades, OR (2) will not have primary course teaching or grading responsibility but will assist an instructor of record.
"YES" indicates the student has met the '18-hours SACS'  requirement  and has primary  responsibility for teaching a course for credit and/or assigning final grades. "NO" indicates the student will only assist instructor and will not be responsible for assigning grades.
